Always carry your Abbott device ID card with you

If you have an Abbott device, it's important to keep your ID card with you at all times when you're away from home. Here's why:

  • In emergency situations, your ID card lets medical personnel know you have an implanted device. It also lists Abbott's toll-free number.
  • When traveling by plane, you'll need to show your card to safely pass through airport security screening devices.
  • The identification card contains information about your implanted device and can help your surgical team identify the appropriate device Instructions for Use and review relevant electrosurgery‑related precautions.
  • The identification card has a reminder to place the device in Surgery Mode.

The above image is an example representation of the Patient Identification Card.  Specific card will vary by device model.

If you need to request a replacement card, contact Abbott Technical Support at 1‑800‑727‑7846, Option 2.
